Hallucination

A hallucination is an AI output that is fluent and confident but factually wrong or entirely invented.

Language models predict plausible text; nothing in the mechanism distinguishes true statements from likely-sounding ones. Fabricated citations, functions, and legal cases are classic examples. Retrieval grounding and human review reduce, but do not eliminate, the problem.
