Serious AI incident
A serious AI incident is a malfunction or harm from an AI system that meets a defined severity bar, which the EU AI Act requires providers to report within strict deadlines.
It turns AI failures into reportable events with a clock attached, much like a data breach. A CISO needs the detection, triage, and reporting path agreed before one happens, not improvised during it.
